How does sleep affect mental health?
Sleep affects mental health in both directions: poor sleep makes worry and low mood harder to handle, and worry and low mood make the next night worse.
When you sleep too little, it is harder to make decisions, solve problems and manage your emotions. Short sleep is also linked to depression4.
Even one sleepless night raises anxiety the next day5. Chapter 3 shows what one short night does to your day.
One bad week will not make you ill. A long run of poor sleep is worth taking seriously.
Can stress and anxiety cause sleep problems?
Yes, stress and anxiety are among the most common causes of sleep problems, because a mind on alert finds it hard to switch off at night6.
Insomnia is when you regularly find it hard to fall asleep or stay asleep, even when you have the time to sleep. Stress, anxiety and depression top the list of its causes6. Chapter 5 explains how to tell if you have it.
Everyone feels anxious sometimes. An anxiety disorder is when fear and worry are strong, hard to control and go on for months. Trouble sleeping is one of its common signs7.
Stress in an Indian home often has many sources at once: a long commute, a demanding job, money worries, a sick parent, a wedding to plan. Each one alone may be fine. Together they follow you to bed.
Shared rooms add to it. In a joint family, the bedroom may be the only quiet space you get all day. That is often when your mind starts on everything you had no time to think about.
Young people feel it too. In a Delhi study of school students, those with signs of depression had 3.5 times the odds of short sleep8. Chapter 4 covers exam weeks.
Happy events count too. Festivals, weddings and a new job also keep the mind busy and the nights late.
Why can't I sleep when I'm stressed or overthinking?
You can't sleep when you're stressed or overthinking because, in the dark with no tasks left, your mind has space to replay the day and plan tomorrow.
Many of us call this "tension". Trying hard to stop the thoughts usually keeps you more awake.
Your body cannot tell a real danger from a worry about an email. Both keep it alert. Your heart beats faster and sleep moves further away.
Worries often feel bigger in the middle of the night. The same worry usually feels lighter in daylight.
One simple step helps many people. Before bed, write tomorrow's worries and tasks on paper. Once it is on paper, you do not have to hold it in your head.
In bed, try slow breathing for a few minutes, with the out-breath longer than the in-breath.
If lying awake has become a habit most nights, Chapter 5 explains how insomnia is treated without pills.
Can lack of sleep cause anxiety or depression?
Lack of sleep is rarely the only cause of anxiety or depression, but a long run of poor sleep clearly raises the risk of both.
Depression is a low, sad or empty mood, or a loss of interest in things, that lasts most of the day, nearly every day, for at least 2 weeks9.
Across 13 long-term studies, people with insomnia had 3.2 times the odds of later anxiety and 2.8 times the odds of later depression10.
A larger review of 34 studies with 172,077 people found the same pattern. People with insomnia had 2.27 times the risk of later depression11.
Over a few days, it shows up faster. Many people notice a short temper, or tears that come easily, after a bad night.
Better sleep and better mood often go together. Treating one often helps the other.
Why does depression make you sleep too much or too little?
Depression can make you sleep too much or too little, because disturbed sleep is one of its common signs9.
Some people wake very early and cannot get back to sleep. Waking very early with a heavy feeling is common in low mood.
The early hours can feel long and lonely, especially when everyone else in the house is asleep.
Others sleep much longer than usual and still feel tired. You may stay in bed late, nap in the afternoon and still have no energy.
Both patterns matter if they last. Poor sleep can have causes that can be treated, such as depression, pain and sleep apnoea12. Sleep apnoea is when breathing stops and starts again and again during sleep; Chapter 1 lists its signs. A sleep clinic can look for these causes.
How can I sleep with anxiety?
To sleep with anxiety, write your worries down before bed, do one calm thing every night, and keep news and work messages out of the last hour.
Choose one calm thing and do it every night: a short walk after dinner, a warm bath, light reading or a prayer. Repeating it tells your body the day is over.
Talk about the worry before bedtime, not in bed. A short talk with your partner, a friend or a family member over evening tea can take a weight off.
Keep the last part of the evening free of news and work messages. Chapter 2 has the full plan for a calm, cool, dark last hour.
If you lie awake for a long time, Chapter 5 explains what to do so the bed does not become a place of worry.
Night shift workers and new parents can use these steps too. The calm routine just moves to whenever your sleep starts.
How can I sleep when someone in my family is ill?
When someone in your family is ill, share the night duties, keep the same wake time each day, and protect one long block of sleep for yourself.
Worry about a loved one is one of the most common reasons for poor sleep. It is normal, and it is not a weakness.
In many Indian homes, one person does most of the night care. If you can, take turns with a sibling, a cousin or a neighbour, so each of you gets a full night some nights.
Sleep when the person you care for sleeps, even for a short time. Keep the room calm and dim at night for both of you.
If you feel low or worn down for 2 weeks or more9, call Tele-MANAS on 14416. You can call for yourself, or to ask how to help the person who is ill.
How common are mental health problems in India?
Mental health problems are common in India: about 1 in 7 adults (13.7%) in a national survey had one at some point, and many got no care3.
The National Mental Health Survey covered 34,802 adults in 12 states in 2015 and 2016. It was run by NIMHANS in Bengaluru3.
Between 28% and 83% of people with a mental disorder were not getting treatment3.
So if you are struggling, you are not alone.
